What is Medical Coding?
At the basic level, medical coders are translators of medical information. They review patient information and assign appropriate diagnosis and procedure codes for patient care, population health statistical tracking and billing purposes.
The healthcare system generates a huge volume of information and these codes provide a system for keeping it all well organized.
It is the process to classify medical and health care concepts using a standardized classification. Inpatient, mortality events, outpatient episodes, general practitioner visits and population health studies can all be coded.
Clinical coding has three key phases: a) Abstraction; b) Assignment; and c) Review.
The process transforms healthcare diagnosis, procedures, medical services etc. into universal medical alphanumeric codes. The diagnoses and procedures codes are taken from medical record documentation, such as transcription of physician’s notes, laboratory and radiologic results, etc.
Clinical coders may have different competency levels depending on the specific tasks and employment setting.
